Student Safety Acknowledgment • I have read and understand the science laboratory safety rules and procedures in this acknowledgment. • My teacher has explained these rules and, where applicable, provided demonstrations or training before I am allowed to participate in laboratory activities. • I understand that I must follow all safety rules and practices at all times, not only during specific activities. • I understand that I may use laboratory tools, equipment, chemicals, and hazardous materials only when instructed, trained, and supervised by the teacher or authorized adult. • If I do not understand how to safely use any tool, chemical, equipment item, apparatus, machine, biological material, or other hazardous material, I will ask the teacher before proceeding. • I understand that failure to follow safety rules may result in removal from the activity, loss of laboratory privileges, parent/guardian notification, or other school consequences.
